Welcome to Matburn Wrestling!

I hereby acknowledge that I voluntarily agree to allow my child to participate in the MatBurn Wrestling Club and its activities and/or any physical activity program offered by, sponsored or supported by, and/or related to the MatBurn Wrestling Club (collectively “MatBurn”).

I understand that all physical activity includes the risk of injury to oneself and that in the case of strenuous exercise the risk is increased. During said MatBurn activities and any events related thereto, including but not limited to pre-event training and/or post-event activities, I assume liability for my childs participation and hold harmless the MatBurn Wrestling Club and/or release the MatBurn Wrestling Club from any and all liability.

Therefore, as consideration for my childs participation in said MatBurn, I hereby hold MatBurn Wrestling Club harmless and waive any and all claims I may have now or at any time in the future, and release MatBurn Wrestling Club, its respective directors, officers, employees, agents, instructors, coaches, contractors, subcontractors, other members of its workforce, and representatives from any and all liability arising from any injuries whatsoever to my child or damage to my childs health, which occur directly or indirectly as a result of their participation in or related to said participation in the MatBurn Wrestling Club, harm of any nature, claims and/or actions that are or may be related to same. I further assume responsibility for any injury to my child or damage to their health now or in the future. I also hold MatBurn Wrestling Club, harmless from and I assume responsibility for any property damage that may occur related to my childs participation.

I understand that this Waiver and Release is binding upon me, my heirs, administrators, executors, successors and assigns.

I have read and understand the terms of this Waiver & Release, I am duly authorized to make these obligations and statements and have signed as my own free act.
